Synopsis - An Ecosystem Of Care: The Integration Of Healthcare And Social Services In Singapore
Research has shown that when health and social service partners work together for the care of patients, it enhances clinical outcomes and improves their quality of life. A multifaceted intervention can also significantly reduce the rate of re-admission and decrease the overall cost of medical care. At the National Council of Social Service, we are committed to ensuring that every person has the opportunity to live a life of dignity to his or her fullest potential within society. As the leader and coordinator of the social service sector in Singapore, we have over 400 member Voluntary Welfare Organisations (VWOs) providing a comprehensive range of social services to those in need.

We will explore the current social service landscape in Singapore and how the continuum of support from health to social care can be more closely integrated. With the growing emphasis on a more holistic and comprehensive approach to care in recent years, we hope to take proactive actions to strengthen the collaboration between healthcare and social care.
