Biographical Sketch


Dr Ng Wai Chong is the Medical Director of Hua Mei Centre for Successful Aging of Tsao Foundation. He is also the Assistant Director of Hua Mei Mobile Clinic. Dr Ng has been practicing home-based primary care since 2000 and his areas of interest include primary health care for the very frail, elder mistreatment, end-of-life care and comprehensive geriatric assessment.

Dr Ng also holds the position of Vice President of Home Health Care Association, and is on the Medical Advisory Board and the GP Advisory Panel of AIC. In addition, he consults for the ACTION Team at Alexandra Hospital (Jurong Health Services).
 


Synopsis

HM 11 - Form follows Function: Age-friendly Community Health Care by Design


With an ageing population, and the bed crunch faced by acute hospitals and residential long term care facilities, there is an urgent need to re-design the way health care is delivered to the population. The needs of an older person and an ageing society will be examined and discussed. Overseas studies and models on age-friendly community health care services will be referenced and reflected upon. A set of principles and practice framework will be presented as a response to the needs of the older persons living in the community.  The service-models developed and practiced at Tsao Foundation’s Hua Mei Centre for Successful Ageing will be used to illustrate the principles and practice framework.
